Preschool Enrollment
This Preschool Enrollment Packet and the Identification & Emergency Information can be completed interactively within the shaded areas and then be printed; or you may print the blank forms to complete by hand. The enrollment packet must be completed in one session. The information cannot be saved as a file for later access. Click on the link above and allow at least one minute for the pages to download. There are 18 pages in the preschool enrollment packet. When the forms are completed, follow the directions on the cover page and take all of the items to the Child Development Center office.
